Abstract
The transition from pediatric to adult care is a major event in the life of an adolescent. It is a gradual process involving the patient, the family and the different healthcare professionals. This step can be more complicated in the case of patients with chronic diseases. For this reason, protocols are being created at the hospital level to reduce the possible complications of an unscheduled transition in patients with special health needs. However, in primary care there are no similar guidelines available and there are very few publications on the subject.
